Skin & Aesthetic Health Assessment

This assessment helps us understand your skin health, dermatological history, current skin conditions or aesthetic concerns, previous treatments, and the medical and lifestyle factors that may influence your response to photobiomodulation. It provides the clinical detail needed to develop a personalized treatment protocol tailored to your skin health, aesthetic goals, and overall health.

Before You Begin

Most people complete this assessment in approximately 20–30 minutes. We recommend setting aside enough uninterrupted time to complete the assessment in a single sitting, as your progress may not be saved if you leave before submitting.

Please answer each question as completely and accurately as possible. The more information you provide, the better we can understand your individual situation and develop a treatment protocol tailored specifically to your needs.

Supporting documents such as dermatology consultation notes, skin biopsy or pathology reports, laboratory results, records of previous skin treatments or cosmetic procedures, and photographs of affected areas can be extremely helpful when reviewing your assessment and developing a personalized treatment protocol. The more information you provide, the more complete the clinical picture we can develop, allowing for a more personalized and comprehensive treatment protocol. If you have any supporting documents or photographs available, we encourage you to upload them. If not, you can still complete and submit your assessment. Simply provide whatever information and records you have available.

Photobiomodulation is intended to support skin health and is not a substitute for appropriate medical care. If you have a new or changing skin lesion, an unexplained bleeding or non-healing area, a suspected skin infection, or a rapidly worsening skin condition, seek medical evaluation rather than waiting for your personalized treatment protocol. If you develop rapidly spreading redness with fever, severe pain, or other signs of a serious infection, seek urgent medical attention.

After You Submit

Once you submit your assessment, Tracy will carefully review your information and develop your personalized photobiomodulation treatment protocol. Because every protocol is individually created based on your medical history, skin health, treatment goals, and supporting information, this process is completed manually and is never automated.

We strive to complete every personalized protocol as quickly as possible. While many are completed sooner, please allow 5–7 business days for your protocol to be prepared. If we need any additional information or clarification while reviewing your assessment, we will contact you by email or text.

Your personalized treatment protocol is delivered digitally and separately from your device shipment. We make every effort to ensure your protocol is delivered before your device arrives, so that when your device is delivered, you'll have everything you need to begin treatment right away.

Once your protocol is complete, you'll receive an email with instructions to access it.
